Immunizations and Physicals

New Students: Per the Florida School Immunization Requirements, No child will be admitted without a thorough physical examination (less than 365 days from the start date of the school) and every child must be immunized according to current Florida State Law. Students entering 7th or 8th grade, must submit proof from their physician of having received immunization Tdap. Proof of immunizations and physical exam are due before entry into MTC. The Diocese of St. Petersburg does not accept any Religious Exemptions.

  1. DH3040 Florida School Entry Health Exam must be dated within the previous 12 months from the entry date into school. This form is different from a sports physical. Sports physicals cannot be used for school entry.
  2. DH680 Florida Certification of Immunization form. The Diocese of St. Petersburg does not recognize religious exemptions for immunizations. The form must be submitted to the school before a child can start the new school year. The Diocese of St. Petersburg does not accept any Religious Exemptions.

New and Current Students: Per the Florida School Immunization Requirements, children entering 7th grade are required to get an addition vaccination before they can attend a Florida school. The name of the vaccination is Diphtheria-tetanus-acellular pertussis (DTaP). You can get this vaccination at your doctor’s office. Once your child receives the vaccination, your doctor will give you an updated DH680 Florida Certification of Immunization form. This is the form that you will provide us with to show proof of vaccination.
