Catholic Schools within the Diocese of St. Petersburg require all students to submit a valid Florida
Department of Health Certificate of Immunization Form DH 680 through their physician as provided for in
Florida Statute 1003.22. Catholic Schools in the Diocese of St. Petersburg do not recognize religious or
conscience objections to this immunization requirement.
Children enrolled at Mother Teresa of Calcutta Catholic school must be up to date on the school immunization requirements. Students entering 7th grade must have Diphtheria-tetanus-acellular pertussis (DTaP) immunization. The forms below are to be provided to the school.

- DH3040 Florida School Entry Health Exam. (dated less than 365 days from the start date of the school)
